HIPAA Notice of Privacy Practices

Healing Haven Centers is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

Treatment. We may use and share your information to provide, coordinate, or manage your psychiatric care, including with other clinicians, pharmacies, and laboratories involved in your treatment.

Payment. We may use and share your information to verify benefits, obtain authorizations, bill your health plan, and collect payment for services.

Health care operations. We may use your information for quality assessment, staff review, scheduling, business management, and other operational activities of the practice.

As required or permitted by law. We may disclose information when required by law, for public health activities, to report suspected abuse or neglect, for health oversight activities, in response to lawful judicial or administrative orders, to avert a serious threat to health or safety, and for other purposes permitted by HIPAA and Florida law.

Other uses and disclosures — including most uses of psychotherapy notes, any marketing use, and any sale of your information — require your written authorization, which you may revoke at any time in writing.

• Inspect and obtain a copy of your health record, including an electronic copy.
• Request a correction to information you believe is inaccurate or incomplete.
• Request confidential communications at an alternative address or phone number.
• Request restrictions on certain uses and disclosures, including a restriction on disclosure to a health plan for services you pay for in full out of pocket.
• Receive an accounting of certain disclosures we have made.
• Receive a paper or electronic copy of this notice on request.
• Be notified in the event of a breach of unsecured protected health information.

Our visits are conducted virtually using secure telehealth tools. Email, text, and web form messages are convenient but cannot be guaranteed to be fully secure; please avoid sending sensitive clinical details through those channels and never use them for urgent needs.

To exercise any right described above, contact us using the information below. If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

We may change this notice and make the revised notice effective for information we already have as well as information we receive in the future. The current notice will always be posted on this page.
